29 CFR 1910.23(b)(8): The employer did not ensure that ladders are used only for the purposes for which they were designed. On or about October 5, 2023, and at times prior thereto, shop floor personnel combined portable A-frame stepladders and wheel cart bases to create a purpose for which the stepladders were not designed, exposing employees to a fall hazard.
