29 CFR 1926.453(b)(2)(v): A safety harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a)DHD Construction, Inc. dba Wall Building Center at 19775 223rd Street, Quinn, SD 57775: On or about and at times prior to September 13, 2023, the employer did not ensure that an employee was protected from falls while conducting commercial roofing activities while working from a Genie aerial lift platform, model number S-45, located on the east side wall of a commercial building. Employees were operating the lift and were not utilizing a personal fall arrest system. This condition exposed the employees to fall and ejection hazards while working from the lift.
