29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers. Location - 61 Dunstar Road, Bedford, MA. On or about August 23, 2023, eight employees were not protected from falling or injury in two instances while working/walking at and near unprotected floor openings. Instance A) First area located at the front section of the house over the new garage. Instance B) Second area was the back of the house were the employees constructed a second floor deck.
