29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Innovative Metals Inc., - Press brake area: The point of operation on the press brakes, including, but not limited to, IC IRON CRAFTER/HYDRAULIC PRESS BREAK ITEM #89 (ORANGE), NIAGARA PRESS BREAK #2 / LJ STEPHENS 12" ITEM # 27 (GREEN) were not guarded to protect employees against pinch points from the point of operation. Occurred on or about 06/07/2023. b) Innovative Metals Inc., - Rollformer area: The point of operation on the roll-former machines, including, but not limited to, LOCKFORMER, MODEL# GEM 3300, S/N SP1182, LOCKFORMER (GREEN), LOCKFORMER (BLUE) Model # 2916, and ROLLFORMER -SEAM PANEL LINE #17 were not guarded to protect employees against pinch points from the point of operation.
