29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of the employees: On or about April 26, 2023: Employee engaged in window replacement and upgrade on a residential home located at 2536 South 191st Circle, Omaha, Nebraska, and was exposed to fall hazards when an employee used a fiberglass extension ladder on the top level of scaffolding as a means of increasing the working level height. This created an unstable working surface and as a result the ladder and scaffolding collapsed, and the exposed employee fell approximately 20 feet to the ground below whereby suffering fatal injuries.
