29 CFR 1910.132(f)(1)(iii):Employee(s) required to use PPE by this section were not trained to know how to don, doff, adjust, and wear PPE: Facility: When a worker was conducting a chop gun operation involving an application of Styrene and fiberglass, Personal Protective Equipment (PPE) was regularly worn in an inappropriate manner. The worker wore a Tyvek suit pulled down to the waist, leaving the face, neck, and arms exposed to airborne Styrene vapors and fiberglass particles.
