29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. On or about February 7, 2023, employees operating SAM 12 machine were exposed to the point of operation. Plastic shields covering the point of operation could be opened, lifted, and/or moved while the machine ran, which exposed employees to caught-in hazards. b. On or about February 7, 2023, employees operating SAM 20 machine were exposed to the point of operation. Plastic shields covering the point of operation could be opened, lifted, and/or moved while the machine ran, which exposed employees to caught-in hazards. c. On or about February 7, 2023, employees operating SAM 36 machine were exposed to the point of operation. Plastic shields covering the point of operation could be opened, lifted, and/or moved while the machine ran, which exposed employees to caught-in hazards. d. On or about March 14, 2023, employees operating SAM 11 machine were exposed to the point of operation. Plastic shields covering the point of operation could be opened, lifted, and/or moved while the machine ran, which exposed employees to caught-in hazards. e. On or about March 14, 2023, employees operating SAM 19 machine were exposed to the point of operation. On or about March 14, 2023, employees operating SAM 23 machine were exposed to the point of operation.
