Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1910.184(c)(6): Slings were not securely attached to their loads: a. Ultimate RB, Inc. - Delphos, Ohio: On or about December 13, 2022, the employer did not ensure that lifting devices were securely attached to the load. Employees that used a 2-legged chain sling and two nylon slings to lift a press mold weighing approximately 2,500 pounds were exposed to struck-by hazards. The nylon slings were not attached to the load in a manner that would prevent the material from being displaced, resulting in an employee being struck by the platen. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
