Text For Citation: 01 Item/Group: 003A Hazard:
1910.147(e) Release from lockout or tagout. Before lockout or tagout devices are removed and energy is restored to the machine or equipment, procedures shall be followed and actions taken by the authorized employee(s) to ensure the following: (e)(1): The machine or equipment. The work area shall be inspected to ensure that nonessential items have been removed and to ensure that machine or equipment components are operationally intact. a) On 12/4/2022 an employee was in the process of making adjustments to the cutting blades of the Rosenthal Shearer-2 (model # WAS-6 HHUJEA), after making the adjustments, with the interlocked safety doors open the employee fed the carpet through the rollers to test the adjustments on the blades, during the feeding process the employee accidently hit the start button on the machine. As a result of the interlocks on the doors not working, this allowed the blades to cycle catching the employee's hand in the blades.
