29 CFR 1910.146(c)(2):The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: a) DOA Line 1 Tower; On or about August 24, 2022 and at times prior, employees were exposed to entrapment and struck-by hazards, in that employees were required to enter the tower on DOA line 1 to perform cleaning and the employer did not post danger signs. b) Ovens on Line 1; On or about August 24, and at times prior, employees were exposed to entrapment, respiratory, and burn hazards, in that employees are required to enter the ovens to perform cleaning and the employer did not post danger signs. c) Ovens on Line 2; On or about August 24, and at times prior, employees were exposed to entrapment, respiratory, and burn hazards, in that employees were required to enter the ovens to perform cleaning and the employer did not post danger signs. d) Dust House; On or about August 24, 2022, and at times prior, employees were exposed to entrapment and struck-by hazards, in that employees were required to enter the dust house to perform maintenance duties and the employer did not post danger signs.
