29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. a) Signature Cabinetry located at 1285 Alum Creek Drive, COLUMBUS, OH 43209: On and preceding August 23, 2022, the employer did not ensure that a forklift equipped with a front end boom attachment used in conjunction with a slab clamp was marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. Forklift operators used a boom attachment with a slab clamp to lift large slabs of granite and quartz that were moved throughout the facility without the weight of the attachments and an accurate load chart posted thereby exposing employees to struck-by hazards.
