29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Baker Industrial Fabrication, LLC, 5660 Iris Pkwy, Frederick, CO 80504: On or about 5/24/2022, the employer utilized a relocatable power tap with a UL listing of UL1363 "Relocatable Power Taps", by using it in a series connection with another extension cord. This exposed workers to fire and electrocution hazards.
