29 CFR 1910.119(j)(4)(iii):The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience: On or about May 9, 2020, in the engine room at Gordon Foods in Houston, TX, employees were exposed to struck-by hazards and chemical hazards of ammonia when the employer did not test pressure safety valves (PSV) on ammonia refrigeration compressors in accordance with the manufacturer's recommendations and good engineering practices. a) The PSV to the oil cooler on compressor TSOC2 was not tested at least every five years. b) The PSV to the oil cooler on compressor TSOC3 was not tested at least every five years. c) The PSV to the oil cooler on compressor TSOC4 was not tested at least every five years. d) The PSV to the oil cooler on compressor TSOC6 was not tested at least every five years.
