29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment. a) On or about March 30, 2022, the employer did not ensure that #40 Waterbury Hand (K15-1) was free from recognized electrical hazards to prevent employees from electrical shock. Employees are exposed to a 120-volt electrical shock during the set up and maintenance of the machine. Specifically, the 6x6 control box exterior housing was not secure by ensuring all knockouts were closed. b) On or about April 12, 2022, the employer did not ensure that Header # 41 (A 96- 1) was free from recognized electrical hazards to prevent employees from electrical shock. Employees are exposed to a 120-volt electrical shock during the set up and operation of the machine for production purposes. Specifically, the 4x4 dual receptacle located near the control panel was missing one receptacle exposing conductors to employees.
