29 CFR 1910.140(c)(11)(i): The employer did not ensure that each horizontal lifeline is designed, installed, and used under the supervision of a qualified person: On or about March 28, 2022, the employer failed to ensure that each horizontal life line was designed, installed, and used under the supervision of a qualified person when a horizontal lifeline was utilized by employees to service the top of tanker trucks exposing employees to a fall hazard of approximately 9 feet.
