29 CFR 1910.179(j)(2)(iii):Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not performed: a. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure the hook on CM 2-ton crane, serial number L-1996NU, model number L, was inspected and documented monthly in that deficiencies identified by a third party company were not repaired. b. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure the hook on the CM half-ton crane, model number F, serial number L 1501 F, was inspected and documented monthly in that deficiencies identified by a third party company were not repaired.
