Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) On and before March 9, 2022, the employer was utilizing a job made attachment to the, but not limited to, Yale model OS030ECN24TE089 order picker forklift. The manufacturer's approval to use the attachment with the lift was not obtained. This condition exposed employees to tip over hazards associated with unsafe forklift operations.
