29 CFR 1910.119(f)(1)(i)(D): The employer did not develop and implement written operating procedures that provide clear instructions for emergency shutdown, including the conditions under which emergency shutdown is required, and the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner: a) In the Machinery Room, on or about December 22, 2021 - The employer did not develop and implement written operating procedures that provide clear instructions for the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner for the recirculators in Recirculator Operations. Failure to assign shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner can expose employees to fire and toxic hazards. b) In the Machinery Room, on or about December 22, 2021 - The employer did not develop and implement written operating procedures that provide clear instructions for the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner for the compressors in Compressor Operating Procedures and Deviation Recovery. Failure to assign shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner can expose employees to fire and toxic hazards. c) On the Machinery Room Roof, on or about December 22, 2021 - The employer did not develop and implement written operating procedures that provide clear instructions for the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner for the evaporative condensers in Evaporative Condensers.
