29 CFR 1910.146(e)(1): Permit-required confined spaces. Before entry is authorized, the employer shall document the completion of measures required by paragraph (d)(3) of this section by preparing an entry permit. a) On or about October 19, 2021, at an establishment located at 112 Mainline Drive in Seymour, Wisconsin, the employer did not complete permit-required confined space entry permits prior to allowing employees to enter Loading Dock 13 during service and maintenance work on dock leveler equipment. b) On or about October 19, 2021, at an establishment located at 112 Mainline Drive in Seymour, Wisconsin, the employer did not complete permit-required confined space entry permits prior to allowing employees to enter Loading Dock 15 during service and maintenance work on dock leveler equipment. c) On or about October 25, 2021, at an establishment located at 112 Mainline Drive in Seymour, Wisconsin, the employer did not complete permit-required confined space entry permits prior to allowing employees to enter Loading Dock 13 during service and maintenance work on dock leveler equipment. NOTE: Appendix D to section 1910.146 presents examples of permits whose elements are considered to comply with the requirements of this section.
