OSH ACT of 1970 Section (5)(a)(1) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees OSH ACT of 1970 Section (5)(a)(1) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that were exposed to chemical, fire and explosion hazards: a) Boulder Scientific Company, LLC., 598 Third Street, Mead, CO 80542: On or about October 22, 2021, the employer failed to provide a work environment free from recognized hazards that were likely to cause death or serious bodily injury. The employer failed to recognize and protect against a fire hazard presented by an impurity that had accumulated in a feed vessel. The routine cleaning step used to prep the vessel was not sufficient in removing the impurity. A less frequently used cleaning procedure was used. The hazard presented by the impurity was not adequately identified in the cleaning instructions. The less frequently used vessel cleaning procedure failed to identify and mitigate the hazard. Abatement Note: Among other methods, a feasible and acceptable abatement method to correct the hazards include: o Revising the batch record to eliminate the cleaning step; o Creating a separate procedure for the vessel cleaning task and designating a minimum frequency for that task; o Maintaining records of the cleaning procedure and its frequency; o Implementing a policy that eliminates all open transfers of flammable liquids.
