29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: The employer is failing to protect employees from fall, struck-by, and crushed-by hazards. This was most recently documented on October 14, 2021, at the residential construction site located at the corner of SW Valley Drive and SW Meadowood Drive, Grain Valley, Missouri, where employees were working in a trench without a safe means of egress. The trench measured approximately 7 feet in depth, approximately 13 feet long, and approximately 9 feet in width.
