29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a) Jobsite located at 2001 Beach Street, Philadelphia, PA- The employer did not ensure employees who were utilizing a job-made platform that was on the forks of a JLG all-terrain forklift was designed, and approved by the manufacturer; thus exposing employees to fall hazards of approximately 30 feet from the ground level. Condition was observed on or about September 8, 2021.
