29 CFR 1910.119(h)(3)(iv):The contract employer did not assure each contract worker follows safe work practices of the facility when performing maintenance of process equipment. On or about July 27, 2021 in the LyondellBasell (LYB) chemical manufacturing facility in La Porte, Texas, workers on the night shift were exposed to process materials, toxic release, and flammable hazards when safe work practices of the facility were not adhered to by contract workers while performing work on the process control valve HV-117 in the Acetic Acid (AA) Unit. a) Pipefitter Supervisor did not adhere to safe work practices while performing work on HV-117 in the Acetic Acid (AA) Unit. b) Pipefitter 1 did not adhere to safe work practices while performing work on HV-117 in the Acetic Acid (AA) Unit. c) Pipefitter 2 did not adhere to safe work practices while performing work on HV-117 in the Acetic Acid (AA) Unit.
