29 CFR 1910.333(a)(2):Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved: On or about July 14, 2021, safety related work practices were not implemented prior to the testing of energized transformers, exposing employees to the hazards associated with an electrical shock. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
