29 CFR 1910.119(d)(2)(i)(A): The employer did not include a block flow diagram or simplified process flow diagram as part of process safety information concerning technology of the process. On or about July 16, 2021, and at times prior thereto, at the tank farm, the employer did not include a block flow diagram or a simplified process flow diagram for crude oil processing. This exposed the employees to fires and explosions hazards.
