29 CFR 1910.178(l)(6) Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a) Worksite, 165 Shore Rd., Glenwood Landing, NY- The employer did not train or ensure that JLG Lull Telehandler operators had received adequate training and the employer did not evaluate powered industrial operator's performance; on or about 6/15/21. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
