29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Production floor, Erema Machines/dosing mechanisms: On March 24, 2021 and at times prior, the employer exposed employees to amputation hazards, in that employees were allowed to perform clearing operations without utilizing the lockout/tagout energy control procedures to prevent accidental startup of the machine. b) Production floor, Cutter Compactor: On March 24, 2021 and at times prior, the employer exposed employees to struck-by, burned-by, and entrapment hazards, in that employees were allowed to perform servicing and cleaning operations without utilizing the lockout/tagout energy control procedures to prevent accidental startup of the machine.
