Text For Citation: 01 Item/Group: 003A Hazard:
Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: (a) On or about March 15, 2021, and at times prior, for the employees exposed to fire and explosions hazards where blades in the Prater Industries, Inc mill used for powdered sugar were not replaced as a set or weighted for balance when blades were replaced, as recommended in the manufacturer's guidance, and resulted in a flash fire occurring in the powdered sugar building at 7525 Red River Road in Wahpeton, North Dakota.
