29 CFR 1910.23(b)(1): The employer did not ensure that ladder rungs, steps, and cleats are parallel, level, and uniformly spaced when the ladder is in position for use. On or about March 11, 2021 and at times prior, at a work platform of the metal sorting line where an inclined trash conveyor passed, the employer did not ensure the fixed ladder rungs were uniformly spaced. When accessing the work platform, employees were exposed to falls from the fixed ladder.
