Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1910.178(l)(3)(i)(A): Powered industrial truck operators did not receive training on operating instructions, warnings, and precautions for the types of truck the operator was authorized to operate: (a) On or about December 10, 2020 - 2200 7th Avenue SW, Cullman, AL, employees were exposed to struck-by hazards when the employee's performance audit was not based on the type of powered industrial truck that the employee was certified to drive. (b) On or about December 10, 2020 - 2200 7th Avenue SW, Cullman, AL, employees were exposed to struck-by hazards when the employees were not instructed to check brakes as described in the manufacturer's manual during the daily inspection. (c) On or about December 10, 2020 - 2200 7th Avenue SW, Cullman, AL, employees were exposed to struck-by hazards when the employees were instructed to not assume that the brakes were working when driving toward objects but were not told that they could not turn around in an aisle which required at least partially placing the powered industrial truck under a rack. (d) On or about December 10, 2020 - 2200 7th Avenue SW, Cullman, AL, employees were exposed to struck-by hazards when the employee's practical instruction did not insure that the employee understood how to stop the powered industrial truck before striking objects in an emergency and nor insure that the employee kept a clear view of the direction of travel.
