29 CFR 1910.178(l)(3)(i)(A): The employer did not ensure that each powered industrial truck operator received training on operating instructions, warnings, or precautions for the types of vehicles that the employees were being trained to operate. a. On or about November 25, 2020, at the above addressed jobsite, powered industrial vehicle (i.e., forklift) operators were not provided with initial training related to the vehicle's proper towing procedures, thereby exposing employees to vehicle tip-over and crushed-by hazards.
