29 CFR 1910.305(a)(2)(x):Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: On or about December 7, 2020 and at times prior and subsequent thereto, in the Eco Services Operations Houston manufacturing facility, workers were exposed to electric shock hazards when flexible cords that were connected into supply outlets were not protected against being damaged while being used to supply electric power to tools and equipment. (a) Flexible cords to portable heaters in the Track 5 & 6 loading area not protected against accidental damage. (b) Flexible cords in the maintenance shop were not protected against accidental damage. (c) Flexible cord to portable heater in the Treatment Services area was not protected against accidental damage.
