29 CFR 1910.178(q)(6): Industrial trucks were altered by the addition of extra parts not provided by the manufacturer: On or about November 30, 2020, employees operating the Clark Forklift, Serial Number G1383016072FA486, were exposed to struck-by and crushing hazards because a die was used as a counterweight and attached to the back of the forklift.
