Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about September 10, 2020 on the jobsite located at 777 Avenue Y, Bushton, Kansas; where the employer failed to inspect and remove from use the damaged personal fall arrest equipment that an employee engaged in scaffolding erection was wearing while exposed to fall hazards up to approximately 27 feet. Instances of damage include but are not limited to: i. Shock absorbers packs on the lanyards damaged, torn and taped closed with duct tape. ii. Labels were not legible on each Nano-Look iii. Nano-Lok had no legible expiration date 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
