29 CFR 1910.146(d)(4): Provide the following equipment (specified in paragraphs (d)(4)(i) through (d)(4)(ix) of this section) at no cost to employees, maintain that equipment properly, and ensure that employees use that equipment properly: 29 CFR 1910.146(d)(4)(i): Testing and monitoring equipment needed to comply with paragraph (d)(5) of this section. a. On or about August 14, 2020, the employer did not ensure that testing equipment was maintained calibrated and utilized by employees entering permit spaces to inspect and prepare the surface for welding. Adequate testing equipment was not provided to identify atmospheric hazards within tanker voids and cavities.
