29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about July 20, 2020, and at times prior thereto, the employer did not visually inspect electrical cords for defects before use on any shift which exposed employees to electrical shock hazards, to include; a) an electrical extension cord was used by employees with the outer jacket damaged and the internal insulation and wires visibly worn; and b) a DeWalt grinder was used with outer jacket damaged/cut with visible internal damage.
