29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) Throughout Facility - On or about July 14, 2020, employees were operating a CAT powered industrial truck (forklift) in an unsafe condition in that operators/employees were exposed to struck-by hazards due to a non-functioning back-up alarm, a non-functioning horn, and a non-functioning parking brake.
