29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allowed, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a)Grangaard Construction, Inc. at the Highway 212 Bridge, Watertown, SD 57201: On or about and at times prior to October 19, 2019, the exposing employer did not ensure that an extension ladder being used for access to the lower level of the coffer dam was extended at least three feet above the upper working level. This condition exposed employees to a fall hazard of approximately 20 feet.
