29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees such as damaged insulation: On or about December 17, 2020, employees were exposed to electrical shock hazard and the employer provided the following damaged parts: a) Northside of shop, Weldwire Positioning Equipment WWRD-30 Drive Rail (Serial Number 172520) with damaged outer insulation and exposed inner wiring; b) Southside of shop, foot pedal for Weldwire Positioning Equipment Model RD-5-INW (Serial Number D-500471JA) with damaged outer insulation and exposed inner wiring; and c) Southside of shop, controller for Preson-Eastin Welding Positioning Equipment Model MA 88LDP (Serial Number MA88LDR-101) with damaged outer insulation and exposed inner wiring.
