29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. On or about 12/12/2019, at the fabrication area, employees were exposed to the point of operation, in that, the Concept 91 MEP Bending Machine did not have a guard at the point of operation. b. On or about 12/12/2019, at the fabrication area, employees were exposed to the point of operation, in that, the small MEP bending machine (mini max 85 V machine), did not have a guard at the point of operation. c. On or about 12/12/2019, at the fabrication area, employees were exposed to the point of operation, in that, the 3 RMS 11 rebar bending machines did not have a guard at the point of operation. d. On or about 12/12/2019, at the fabrication area, employees were exposed to the point of operation, in that, the Arnold 8 rebar bending machine did not have a guard at the point of operation. e. On or about 12/12/2019, at the fabrication area, employees were exposed to the point of operation, in that, the RMS 14 rebar bending machine did not have a guard at the point of operation. f. On or about 12/12/2019, at the fabrication area, employees were exposed to the point of operation, in that, the CRS 18 rebar bending machine did not have a guard at the point of operation. g. On or about 12/12/2019, at the fabrication area shear line "1", employees were exposed to the point of operation, in that, the guard at the point of operation was tied in the up position exposing employees to the moving parts.
