29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On November 7, 2019 at the Kersten Lumber Co. Inc. facility located at 295 Hwy 45 Birnamwood; the employer failed to clearly identify all of the specific steps to be followed by employees to isolate and disable the De-Barker, Head Saw, Re-Saw, and conveyor equipment in order to safely de-energize the equipment and control the mechanical hazards and to prevent unintentional or accidental machine start up. The current procedures lacked the following: 1. The purpose of the energy control procedure, 2. Identify and locate nominal electric disconnect switches to control isolation, 3. Sequential steps to be followed to protect employees from mechanical hazards, 4. Application of person lockout devices, 5. Identify residual energy has dissipated (e.g. blade stopped before guards removed), 6. Verify and test that de-energization and isolation has been accomplished. 7. Provide steps for restart.
