29 CFR 1910.119(e)(3)(i): The process hazard analysis shall include the hazards of the process: At a facility located at 191 W. 28th Avenue, Oshkosh, WI 54902; the employer's process hazard analysis (PHA) did not include the hazards of the process. The employer's 2014 and 2019 PHAs did not analyze if flash fire hazards existed inside the converter room were anhydrous ammonia (flammable) was present in the process equipment.
