Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: The employer is failing to protect employees from electrocution hazards associated from a circuit breaker which was not locked out during installation of a new light fixture between the hot and the cold water liquid storage tanks. This was most recently documented on August 9, 2019 at the jobsite located at 1607 Anaconda Road, Harrisonville, Missouri. An employee was shocked and fatally injured while working with a 110 volt energized conductor that was not rendered inoperative or guarded and was not tagged.
