29 CFR 1910.147(d)(2): An orderly shutdown of the machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage: At 1120 Mason Circle S., the orderly shutdown of the Normac #1 Grinding Machine, m/n BS-35, was not performed in that the grinding wheel was still spinning (kenetic energy) when the authorized employee opened the door (guard) and placed his left hand into the danger zone. This action resulting in an amputation on or about 07/15/19. This violation was noted on 07/23/19.
