29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above uncovered or unguarded dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system: a) Worksite, Conche Room: Employees operating the McCarter Iron Works 20,000 lb. horizontal tank/chocolate melter were exposed to hazards including but not limited to engulfment and rotating parts in that the opening over which employees stood was not covered or guarded and employees were not protected from falling into the dangerous equipment; on or about 7/9/19. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
