29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about June 25, 2019 and at times prior thereto, a 3-wire flexible cord (extension cord) with a broken ground prong was plugged into a 2-wire flexible cord and being used to power a metal frame fan, exposing employees to the hazard of electric shock.
