29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained: The employer is failing to protect employees from electrical shock hazards when performing well and pump maintenance operations. This was most recently documented, on or about 6/12/2019, at the jobsite located at Rd N 80 (39.466304, -100.575538) Selden KS 67757. An employee operating a truck mounted Smeal 6T derrick raised the derrick structure within 10 feet of an energized overhead powerline.
