29 CFR 1910.147(e): Release from lockout or tagout. Before lockout or tagout devices are removed and energy is restored to the machine or equipment, procedures shall be followed and actions taken by the authorized employee(s) to ensure the following: (e)(2)(i): The work area shall be checked to ensure that all employees have been safely positioned or removed. a) On or about April 16, 2019, the employer did not ensure that procedures and actions were taken by the employee before the Worthington welding positioning machine #33112238 was released from lockout and energy restored to the machine in that assurance was not made that the work area was clear and all employees had been safety positioned or removed.
