29 CFR 1910.179(j)(2)(iii): Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not performed: (a) Deshazo 20 Ton Top Running Single Girder Crane - On or about April 02, 2019 the last documented monthly the crane hook was completed on March 14, 2018. The required inspection of the crane hook identifies any damage to the hook of the crane that can fail during regular usage. Employees were exposed to struck-by hazards when it is not properly inspected.
