Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about December 27, 2018, employees were exposed to pinch point and rotating part hazards in that Mill #3 was not locked out prior to servicing tasks such as, but not limited to, cleaning, greasing, blade changes, and setup of the machine. The employer did not ensure that energy control application steps were implemented to control the machine's gravitational and electrical/mechanical energy sources prior to the servicing tasks - the employer did not: - Ensure employees had the method or means to control hazardous energy. - Affix lockout device(s) to control the energy source(s). - Verify that isolation and de-energization of the machine or equipment had been accomplished.
